Annual Community Health Dashboards

Each year, the public health branch is committed to providing an annual update on the state of health in Sutter County. This helps us analyze what health problems community members experience and monitor progress towards improving the health and well-being for all Sutter County residents. These updates include an annual health status snapshot and individual community health dashboards on various health topics.

Please see below for the Annual Health Status Snapshot and community health dashboards.

Below is a description and information included in each dashboard:
  • Chronic Diseases - Long lasting or recurrent medical conditions such as heart disease, diabetes, and chronic lower respiratory diseases.
  • Communicable Diseases - Illnesses that spread from one person to another through direct contact or something else (insect or animal bites, contaminated surfaces, or through the air). This includes tuberculosis, chronic hepatitis C, and sexually transmitted infections.
  • Health Risk Factors - Behaviors, characteristics, or exposures that increase the likelihood of a person developing a disease or health problem. Examples of factors include a lack of physical activity, tobacco use, poor nutrition, homelessness, and adverse childhood experiences.
  • Maternal and Child Health - Maternal health refers to health during pregnancy, childbirth, and postnatal (after childbirth). Measures includes perinatal (time during pregnancy to after birth) mood disorders and anxiety disorders, breastfeeding rates, and infant mortality rates.
  • Population & Geographic Overview - A breakdown of our Sutter County population by age, race/ethnicity, disability, education, and more.

We also have the Community Health Improvement Plan (CHIP) document, which is a guiding document for the next 3 to 5 years to address some of these health priorities. Please click the CHIP tile below to learn how we are improving health and well-being in Sutter County.
