
Our Case Management team is focused on providing wrap around services for participants. Case Managers are responsible for working one-on-one with the participants to meet their needs. Participants can receive assistance with the following: navigating the social services system, establishing a primary care physician, referrals to behavioral health services, treatment for substance use disorders, employment and education services, help with budgeting and managing finances, and housing. Case managers adapt to their client's needs to ensure that their individual plan will successfully lead them to housing.
